Appointments can be made with Mr Jason Rockey for Bendigo by contacting us on 03 5442 1150. Alternatively our friendly receptionists will contact patients directly upon receiving referrals via fax, email or post.

Following a consultation with Mr Jason Rockey, any necessary procedures can then be scheduled with Mr Jason Rockey at Castlemaine Health, St John of God Hospital Bendigo, or Northpark Private Hospital.

Please ensure you have the following items with you on the day of your appointment:

• Current referral

• A list of all current medications

• Relevant scans & reports

• Medicare Card

• Pension or concession card

• Private Health insurance details

All fees are payable on the day of your consultation. Medicare rebates apply with valid referrals
